I have never experienced Eli having white hands, even in the CD version of the game.
This needs to be investigated! I seen it with my own eyes. Even in Sven Co-op it does it sometimes.
The game is hardcoded to change the skingroup whenever the head is Luther. If anything were to defy this, it would be a custom model with a broken/absent skingroup or, in the possible case for Sven, something lost over networking.
When they first introduced the Luther head, they lacked any skingroup for the model as they reskinned an existing head.
Why are the photos on Combine Overwiki and the Valve wiki both showing it?
A. Overwiki appears to depict it correctly on the Scientist page. B. Someone made the images in HLMV or some other model program without manually switching the skingroup.

In the Half-Life: Source model sources, the Alien Grunt, Nihilanth, and the Loader contain decompiled resources. Only the Loader uses decompiled content for building (QC and possibly animations.)

Alien Slave leave bullet holes where their electric attack touches the world. In Half-Life: Source it was changed to a scorch decal but on the store page photos you can see it still using the bullet decal. Half-Life 1 also contains scorch decals.
During Half Life 2's development the developers couldn't get one of Grigori's sequences to play for some reason. This issue eluded them for so long that they ran out of time for bug fixing and opted to hard-code the sequence into the game, as in "if it's this map at this time, just play the damn thing so we can all move on with our lives" hard-coded.

Unfortunately I have no clue, not even sure if they mentioned the specific one where I heard it from. IGN got some HL2 developers to do a commentary over a speedrun around HLA's release and while the speedrunner was in Ravenholm one of them mentioned that they had to do that.
